OAKLAND -- The U.S. Chemical Safety and Hazard Investigation Board (CSB) will convene a Board of Inquiry as part of its investigation of the deadly Tosco Avon Refinery explosion in February.The session will be held Aug. 18 at the Contra Costa County Board of Supervisors Chambers, County Administration Building, 651 Pine St. in Martinez. During the inquiry, CSB will present the findings of its preliminary investigation of the incident, in which four workers died and a fifth was seriously injured.
