OAKLAND -- California's Division of Occupational Safety and Health (DOSH) has issued 33 citations to Tosco Refining Co., with proposed penalties of more than $800,000, for safety violations in a deadly February flash fire in Martinez. The Cal/OSHA action is the highest penalty amount ever issued against a single employer, Cal/OSHA Chief John Howard told COR.Four workers died and another was critically injured in the Feb. 23 fire
