This appeal established a serious violation of 2395.51(a). The proposed penalty of $650 was reduced to, and affirmed at, $200. Nonserious violations of 2340.14(b), 2360.7(b), 2340.12(a), and 2395.83(b) were affirmed. The $60 penalty proposed for the violation of 2360.7(b) was vacated. No other penalties were proposed. Nonserious violations of 1766(c), 1763(a) and 1767(a) were dismissed.
