An alleged serious violation of 3314(a) (cleaning, repairing and adjusting machinery, prime movers and equipment) was vacated. Employer, a concrete pipe manufacturer, was charged with not stopping a portable welding machine during repair. An employee was injured when his hand was drawn into the cooling fan. He had sprayed stuck parts with lubricant. Neither he nor any other percipient witness testified at the hearing.
