CAL. CODE REGS, tit. 8, § 3314(a) (2002) - Two employees were injured when a hose for spraying fireproofing was disconnected while still clogged and pressurized, and the hose whipped around. The violation was established, but the Division did not prove to a substantial probability that it would result in serious physical harm, so the classification was reduced from serious to general.
