CAL. CODE REGS, tit. 8, § 4552(b) (2003) - Employer failed to enforce a requirement that a pusher or tamper be used to feed wheat grass into a juicer, and an employee who used her fingers instead suffered a fingertip amputation, a serious, accident-related violation. The juicer was shown to be a grinder and came within the cited safety order's coverage.
