INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – SYSTEM FOR COMMUNICATING WITH EMPLOYEES
Cal. Code Regs, tit. 8, § 3203(a)(3) (2016) – Employer failed to effectively implement its IIPP. Employees’ reports of hazards were ignored, in contravention of Employer’s communication policy and the safety order.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – PROCEDURES FOR IDENTIFYING AND EVALUATING WORKPLACE HAZARDS
Cal. Code Regs, tit. 8, §§ 3203(a)(4)(B) and (a)(4)(C) (2016) – Employer failed to identify and evaluate workplace hazards associated with defective, propane-fueled forklifts.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – PROCEDURE TO INVESTIGATE OCCUPATIONAL INJURY OR OCCUPATIONAL ILLNESS
Cal. Code Regs, tit. 8, § 3203(a)(5) (2016) – Employer failed to investigate occupational illnesses experienced by its employees who were working at a secondary employer.

INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – TRAINING
Cal. Code Regs, tit. 8, § 3203(a)(7)(E) (2016) – Employer failed to adequately to train and instruct its employees, or to ensure that its employees were properly trained by a secondary employer, with respect to hazards posed by exposure to carbon monoxide at the secondary employer’s worksite.

DUSTS, FUMES, MISTS, VAPORS AND GASES – AIRBORNE CONTAMINANTS, WORKPLACE MONITORING
Cal. Code Regs, tit. 8, § 5155(e)(1) (2016) – Employer failed to monitor the level of carbon monoxide, where forklifts powered by internal combustion engines were operated in an enclosed, unventilated space, and symptoms of exposure to carbon monoxide had been exhibited by exposed employees. The violation was classified properly as willful.

DUSTS, FUMES, MISTS, VAPORS AND GASES – AIRBORNE CONTAMINANTS, EXPOSURE LIMITS
Cal. Code Regs, tit. 8, § 5155(c)(3) (2016) – Employer’s employees were exposed to carbon monoxide levels above the established ceiling limit of 200 ppm. The violation was properly classified as willful, accident-related.
