ERECTION AND CONSTRUCTION – REINFORCING STEEL AND OTHER PROJECTIONS, PROTECTION OF EMPLOYEES AGAINST HAZARD OF IMPALEMENT
Cal. Code Regs, tit. 8, § 1712(c)(1) (2016) – Employer failed to ensure that employees working at grade or at the same surface as exposed protruding steel anchor bolts located less than six feet above the working surface were protected from impalement. A serious, but not willful, violation existed, because Employer appeared to have had a mistaken belief as to how to comply with the safety order. 

ACCESS, WORK SPACE AND WORK AREAS – PORTABLE LADDERS, PROHIBITED USES
Cal. Code Regs, tit. 8, § 3276(e)(16)(C) (2016) – Employer willfully failed to ensure that portable stepladders were not used as a single ladder in a closed position.
