EXCAVATIONS, GENERAL REQUIREMENTS – ACCESS AND EGRESS, MEANS OF EGRESS FROM TRENCH EXCAVATIONS
Cal. Code Regs, tit. 8, § 1541(c)(2) (2016) – Employer failed to provide a means of egress from a trench excavation so as to require no more than 25 feet of lateral travel for employees.

EXCAVATIONS, GENERAL REQUIREMENTS – PROTECTION FROM HAZARDS ASSOCIATED WITH WATER ACCUMULATION
Cal. Code Regs, tit. 8, § 1541(h)(1) (2016) – Employer failed to take adequate precautions to protect employees against the hazards posed by water accumulation. The pumps that Employer used were not adequate, given the continued presence of water in the trench.

EXCAVATIONS, REQUIREMENTS FOR PROTECTIVE SYSTEMS – PROTECTION OF EMPLOYEES IN EXCAVATIONS
Cal. Code Regs, tit. 8, § 1541.1(a)(1) (2016) – Employer failed to protect employees in an excavation from cave-ins by an adequate protective system designed in accordance with §1541.1(b) or (c). The Board sustained the serious, accident-related and willful characterizations of the violations at issue.
