INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – IDENTIFYING AND EVALUATING WORKPLACE HAZARDS
Cal. Code Regs, tit. 8, §§ 6507, 3203(a)(4), 3203(a)(7)(C) and (F) (2015) – Employer’s IIPP failed to include procedures for identifying and evaluating workplace hazards to identify unsafe conditions and work practices; specifically, the removal of pins while raising an oil rig mast.

MACHINERY AND EQUIPMENT – HAZARDS CAUSED BY BREAKAGE, RELEASE OF MECHANICAL ENERGY, OR LOOSENING AND/OR FALLING
Cal. Code Regs, tit. 8, § 3328(e) (2015) – The Division did not establish that Employer failed to secure machinery and component parts to minimize hazards caused by breakage, release of mechanical energy (e.g., broken springs), or loosening and/or falling.
