CONFINED SPACES – WRITTEN PERMIT SPACE PROGRAM
Cal. Code Regs, tit. 8, § 5157(c)(4) (2014) – Employer failed to provide its written permit space program for a reservoir that qualified as a confined space.
CONFINED SPACES – PERMIT SYSTEM
Cal. Code Regs, tit. 8, § 5157(e)(1) (2014) – Employer failed to document an entry permit prior to its divers’ entry into a reservoir, via a hatch.
CONFINED SPACES – RESCUE AND EMERGENCY SERVICES
Cal. Code Regs, tit. 8, § 5157(k)(1) (2014) – Employer failed to provide required training to designated members of its rescue service. The violation was properly classified as serious.
DIVING OPERATIONS – HOSE-TENDING REQUIREMENT
Cal. Code Regs, tit. 8, § 6056(b)(2)(C) (2014) – The Division failed to establish that Employer did not have one surface-supplied diver at the underwater point of entry in the reservoir for the purpose of hose-tending.
