44 COR 40-7947 [¶22,783] PERMIT REQUIREMENTS – WORK ACTIVITIES SUBJECT TO PERMIT REQUIREMENTS AND TYPES OF PERMITS REQUIRED Cal. Code Regs, tit. 8, § 341(d) (2017) – Employer failed to obtain a project permit or annual permit before engaging in excavation work. CONSTRUCTION SAFETY ORDERS – INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) Cal. Code Regs, tit. 8, § 1509(a) (2017) – Employer failed to maintain records showing that it trained employees on hazards related to their job duties, or provide evidence of an IIPP to the Division upon request. PERSONAL SAFETY DEVICES AND SAFEGUARDS – HEAT ILLNESS PREVENTION Cal. Code Regs, tit. 8, § 3395(i) (2017) – Employer failed to provide evidence of a heat illness prevention plan (HIPP) to the Division upon request. CONSTRUCTION SAFETY ORDERS – WASHING FACILITIES Cal. Code Regs, tit. 8, § 1527(a) (2017) – Employer failed to provide adequate handwashing facilities to employees at the worksite. CONSTRUCTION SAFETY ORDERS – EXCAVATIONS, INSPECTION CONDUCTED BY COMPETENT PERSON Cal. Code Regs, tit. 8, § 1541(k)(1) (2017) – Employer failed to have an excavation inspected by a competent person. CONSTRUCTION SAFETY ORDERS – EMERGENCY MEDICAL SERVICES Cal. Code Regs, tit. 8, § 1512(b) (2017) – Employer failed to ensure that it had employees at a worksite who were appropriately trained to render first aid.
