INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – NEW OR PREVIOUSLY UNRECOGNIZED HAZARD
Cal. Code Regs, tit. 8, § 3203(a)(4)(C) (2015) – Employer failed to conduct an inspection of a new or unrecognized hazard involving an employee who was tasked with working on a utility pole at 25 feet above ground, in adverse weather.

TELECOMMUNICATION SAFETY ORDERS – INCLEMENT WEATHER
Cal. Code Regs, tit. 8, § 8602(i) (2015) –  Employer failed to discontinue work from structures when adverse weather made the work unsafe. The ALJ reclassified the violation from serious to general, but found the penalty duplicative and vacated it.
