INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – CODE OF SAFE PRACTICES (CSP)
Cal. Code Regs, tit. 8, § 1509(b) (2016) – Employer failed to provide a CSP in response to the Division’s request, or explain its failure to do so at the hearing.

HEAT ILLNESS PREVENTION PROGRAM (HIPP) – TRAINING
Cal. Code Regs, tit. 8, § 3395(f)(3) (2016) – Employer failed to provide its HIPP in response to the Division’s request, or explain its failure to do so at the hearing.

DUST, FUMES, MISTS, VAPORS AND GASES – RESPIRATORY PROTECTION PROGRAM
Cal. Code Regs, tit. 8, § 5144(c) (2016) – The Division failed to establish that Employer was required to develop and implement a written respiratory protection program. It offered no evidence that respirators were either necessary to protect any employee at the worksite, or required by Employer.

ACCESS, WORK SPACE, AND WORK AREAS – PORTABLE LADDERS, CARE, USE, INSPECTION AND MAINTENANCE OF LADDERS, FOOTING SUPPORT
Cal. Code Regs, tit. 8, § 3276(e)(7) (2016) – Employer violated the safety order by permitting the employee to climb a ladder that was not placed on secure and level footing, a serious, accident-related violation.
