INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) – TRAINING
Cal. Code Regs, tit. 8, § 3203(a) (2015) – Employer violated the safety order by failing to effectively implement inspection procedures to uncover a workplace hazard related to a dough-divider machine. Employer further failed to train employees on how to lock out the divider during cleaning and ensure that supervisors were familiar with the safe operation of the divider and the hazards attendant to cleaning it.

CLEANING, REPAIRING, SERVICING AND ADJUSTING PRIME MOVERS, MACHINERY AND EQUIPMENT, INCLUDING LOCKOUT/TAGOUT – PERIODIC INSPECTION BY AUTHORIZED EMPLOYEE
Cal. Code Regs, tit. 8, § 3314(j)(1) (2015) – Employer violated the safety order by by failing to train employees on procedures and hazards attendant to cleaning the dough-divider machine.

POINTS OF OPERATION AND OTHER HAZARDOUS PARTS OF MACHINERY – GUARDING REQUIRED AT POINT OF OPERATION
Cal. Code Regs, tit. 8, § 4184(b) (2015) – Employer violated the safety order by failing to guard the dough-divider machine’s cutting mechanism.

CLEANING, REPAIRING, SERVICING AND ADJUSTING PRIME MOVERS, MACHINERY AND EQUIPMENT, INCLUDING LOCKOUT/TAGOUT – CLEANING, SERVICING AND AND ADJUSTING OPERATIONS
Cal. Code Regs, tit. 8, § 3314(c) (2015) – Employer violated the safety order by failing to de-energize or disengage the dough divider’s power source while it was being cleaned. The violation caused a serious injury to its employee.
