PENALTIES - FAILURE TO REPORT SERIOUS INJURY
Cal. Code Regs, tit. 8, § 342(a) (2010) - Employer failed to report a serious injury timely, but was entitled to penalty reduction. 

CRANES - FAILURE TO PROPERLY RIG LOAD DURING HOISTING OPERATION
Cal. Code Regs, tit. 8, § 4999(b)(1) (2010) - Employer’s supervisor used a hand-held control box to operate an overhead traveling crane to hoist a load of bundled rebar and land it in an upright rack.  Employer failed to properly attach the load since slings were not used to attach the hooks on a strong back to the load during the hoist. 

CRANES - SLINGS
Cal. Code Regs, tit. 8, § 5042(a)(4) (2010) - Employer attached a load of rebar to the rigging by hooking rigging chains to the bundling wire around the rebar load.
